Dear Patricia. Mike and Dave.
Thanks for the spreadsheet about GHW funding. Just a few more clarifications will help to ensure that all our fundraising
efforts are complementary and well coordinated with no duplication and inadvertent competition.

i.

I gather you have received 84000$ and are expecting around 30000$ if all the proposals show in the column 2005
are received. Incidentally exchange 12000$ is shown twice. Are they two proposals?

ii.

How long can you function in GHW secretariat/GHW process with this?

iii.

In many agencies you have mentioned “does not fall into their programme areas”. Can you ask the person who
contacted them whether there would be interest in those organizations to support PHA 2 (Perhaps a workshop or
travel grants) or other PHM activity / event. In which case we (Maria, Andy and I) need to have contact details about
whom to approach for that follow up.

iv.

In the original project proposal drawn up by GEGA / Medact/PHM, we (PHM) were to make a contribution
(through fundraising). Is there still an expectation. If so how much in the current context?

v.

We are (PHM) in a rather precarious condition but some grant approvals are coming through. Do you have enough
to give us a 5000$ loan till Feb. 2003 if it is urgent or is required. We may not be able to transfer money but we
may before the end of the project support some travel or advocacy costs etc - perhaps!!

vi.

What are your plans for IHF WSF? An update is being sent out by us today. Is GHW planning to use the
opportunity for a discussion on the final chapter etc which we had suggested in one of the teleconferences?

vii.

We have a grant from Actionaid Bangkok specifically earmarked for sterengthening Asian PHM parti cipation in
WSF. We may support Deien, Cheekhoon, Qasem. Amit etc., to attend IHF/WSF with that grant and that would
greatly help any GHW process there. Prem and Unni are also attending

viii.

For the time being Patricia’s travel for PHA 2 IOC meeting will have to be borne by GHW as contribution to the
process. Perhaps when our PHA 2 / PHM grants come through later in the year, we could do a reimbursement
transfer if that is found to be necessary

At present the only crossovers are Christian Aid and IDRC, MDM and MSF! What have you asked them for and how much?
Sorry about all these questions but I am just working on consolidation of 2005 budget for PHM and these clarifications would
be helpful.

iii) As Mike said - Not till after I am back from Australia on Jan 9.

Regarding Christian Aid. MDM and MSF! The huge list 1 sent you are
options and we have not approached any of them where it is not indicated
otherwise. We will be sure to keep in mind who PHM is approaching. Would
be good to have a spreadsheet of where you are going so we can keep track
of tliis.
Regarding the I HF in Porto Alegre. We thought to ask Amit to lead the
plenary session on the GHW for us. Armando also mentioned a two hour
workshop also- maybe Amit and others can lead a discussion on
recommendations for some of the chapters and on athe broader
recommendations as Dave mentioned. Armando contacted me yesterday about
this and I have not had a chance to reply.

It's remiss of me I know but I'm afraid I didn't open the original spreadsheet that Pat sent me and I now can't find it in my
messages. So working without my glasses I'll tell you where we are at:
i. In total for the Global Health Watch we have so far received $130,000 which is all being spent on the production process.
We have a new budget for the advocacy proposal which amounts to a similar figure. There are NO absolute committments of
money to this advocacy proposal. Exchange has given us some funding already ($18,000), and we have asked them for some
more funding for the advocacy component (around $20,000 as I recall).

ii. Officially we have guaranteed secretariat resources ONLY until the end of February. We are fundraising for a functionning
secretariat which would include Pat until December, and me on a consultancy basis to the end of July.
iii.

Pat may be able to do this but noi until she is back from Oz in the second week of January

iv. No - we are not expecting anything - we know you are in a precarious position. However, I think it was agreed in Mexico
that for example the Ecuadoreans would approach IDRC independently for funds to launch the Watch in Cuenca. Cant think
of any more synergies at the moment, but maybe you are putting something in your proposal.
v. From Medact’s perspective the loan is out I'm afraid. Of the $130,000 mentioned above 90% will have been spent by end of
February - and we don't have ANY ABSOLUTE guarantees of more money, although we are hopeful. And Medact’s reserves
are so limited (just over $20,000) that it is simply too big a risk for us to carry to make a loan. Really sorry about this.

All I would add is that there is a GEGA proposal that includes a section for GHW that is with SIDA. But so far we have only had
silence from them.
In terms of the recommendations, the truth is that we are way behind schedule on most things. There is a draft set of
recomemndations related to the political determinants of health - this will cover the reform of the WB/IMF/WTO; debt
cancellation; aid: corporate tax; trade reforms etc. This could be circulated and discussed in Porto Allegre - however, I think we
would want to be careful not to have this circulated too widely that is becomes publicly accessible.
There will also be a number of individual chapters which will incorporate some recommendations - and these could be
discussed as well. I would suggest focussing on the water, food / agriculture and the war / militarism chapters. This should
probably be sufficient??
In tyerms ofthe health care sector chapters, need more time on this. Have a meeting with David Sanders in January on this we are trying to raise money to bring out either you, abhay or ravi to be part of this meeting.

Once we get everything to printers late feb / early march, we will be able to also start thinking about fundraising for the next
version ofthe Watch - if we manage to get good publicity with the launch etc. then we should be ready with a proposal to hit
donors in July.
Ravi - it would also help if you could put out a call to all in PHM to consider volunteering to organise local launches of the
GHW in July. We need to have at least one launch in the middle east, north america and far east.

Service*-Vicente Navarro’s journal. The International Journal for Equity in
Health has a small circulation, but might get good reaction. Also, the
Srnzilic-i Journal of Medical Education would probably be sympathetic to the
cause. If you’re interested in reaching social justice journals, the list

TURNING TO ADVOCACY

We are nearing the end of the first stage of our Global Health Watch
journey - the publication will go to press at the beginning of March. As it
does, we want to alert you to the next stage of our work - ADVOCACY.
In many respects this is the key component of the Watch. This is how the
publication will become noticed; this is also how we can expand and glue
together the individuals and movements who have contributed with those who
did not.
The last chapter will point at the areas that need to be looked at and
prioritise some recommendations. We will spend more time on honing our key
messages in the accompanying summary document that will be produced by
mid-May. The main book will link to this summary document.

We want to encourage a broad participation in the production of the summaiy
document - in the end we want this document to contain some strong
statements that as many health and health-related actors can campaign around
as possible; as well as containing key advocacy messages we want to project
to the world at the time of PFIA 2

A draft will be available by the end of March, and then we will give a month
for consultation about the key messages with a wide range of interested
actors.

Under-nutrition seems to be inexplicable in a world where the food market
ascends to the 11% of the global trade and food prices have decline over the
last years. Nevertheless it is one of the most important causes of illness
and death globally as well as a key factor in poverty reproduction.

This chapter looks at the underlying causes of under and over nourishment
both in developing and developed countries as directly related to the
globalisation and liberalisation processes that have been taken place in the
last decades. By implementing global marketing strategies and liberal
programmes such as NAFTA, small local producers have been displaced from the
market leaving the food system under the control of few global corporations
based in developed countries. Thus generating poverty and inequalities as
well as a decline in agricultural and rural investment in developing
countries, not mentioning the threat posed by the growing market of
genetically modified seeds.
Another dimension of the problem closely intertwined with the latter, is the
globalisation of diets around the globe. This process has been encouraged
to a great extend by international organisms such as the World Bank (which
offers loans in order to develop certain type of food industry) as well as
the entry of large food multinationals and retailers. The consequences are
manifold, there is a cultural impact in terms of the progressive shift from
diverse traditional local food towards a westernised homogeneous diet and on
the other hand problems of over-nutrition and obesity are becoming new
public health burdens. These are difficult to overcome not only because of
the complexity of the problem itself but because of the pressure exerted
from big food companies.
Towards the end of the chapter various strategies to tackle the problem are
proposed, such as the development of international standards and national
legislation in order to protect and promote national food security. Finally
there is a call civil society to take a more active role in restoring food
to the status of a human and cultural right.

GHW AT THE WORLD SOCIAL FORUM

The fifth edition of the World Social Forum was held between 26 and 31 last
month. This was the biggest ever edition of the Forum and congregated more
than 150,000 participants in the Brazilian City of Porto Alegre. The
enormous gathering of groups and movements from all corners of the world was
an inspiring opportunity to debate, share experiences and network.

One of the GHW Secretariat members, Claudia Lerna, attended the WSF last
week. She facilitated a workshop organised by IFRHHO (International
Federation of Health and Fluman Rights Organisations) and chaired by the UN
Special Rapporteur for the Right to Health discussing strategies to use
Millennium Development Goals as tools for the achievement of the right to
health. She also had meetings to discuss the Global Health Watch initiative
with the representatives from ALAMES (Latin American Association of Social
Medicine), Action Aid and the European Network for the Right to Health, as
well as the members of the various chapters of the Peoples Health Movement.

A note from the People’s Health Movement, GEGA and Medact

Dear Reader,

Oae-aad-a-half years agoTwo years ago, our three organisations came together in recognition of the

fact that health workers and civil society groups needed an instrument to report, not so much on the
state of global health, but on the state of action towards improving health and reducing the
unconscionable disparity in health between different peoples.

In other words, it was time for a report to be written about the performance and actions of the very

institutions that normally write ‘global reports’! It was time to turn the tables by assessing and reporting

on the performance of international health agencies such as the World Health Organisation and
UNICEF, donor agencies, rich country governments, the World Bank, the IMF and WTO. It was time

for an alternative world health report that would confront the political roots of poor health and inequity

and show up the humanitarian rhetoric that promises so much but delivers so little. The result is a first
edition of the Global Health Watch which unmasks the gap between rhetoric and reality.

We also believed that an-alteFnative-woFld-health-FepoFtsuch a report should-could offer a platform that

to brings together the-various-civil society and non-governmental-organisations and individuals
working in health and health-related different-sector-that-nonetheless-atfeGhhealtsectorsh: these
include those working on gender discrimination, global trade an4-econom-iG&r-the-clebt-buFden7

environmental protection, access to water and food, arms trade, militarism, conflicts and disastersand
on-the-ams-tFade-and-militan-sm. Global Health Watch 2005 has achieved this, and we hope to see

improved collaboration between these different actors into the future.

But-we-also-aoknowledqe-manY-fimitations to-this-repoFtWe would like to acknowledge some

limitations. We have endeavoured to involve people from as many countries as possible, but have not
succeeded to ensure adequate input from many regions, including the middle-Middle east-East and

chinaChina. We have tried to cover most of the key issues relevant to health, but we have left out a

discussion of many important topics. -The-T-sunaffM-thaVaf-feGted-hundFeds-oMhou-saRds-oTpeople-in-so

responses-tc-nat-wal-and-Gther-disa-ster-s-aFe-managed-aAd-GOOFdinated—bat-this-is-missing-in-the

repodr-With a limited budget and a tiny secretariat, we-have-not-always-been-able-to-give-eaGh-chapter
the-careful-attention-it-deserveswe were simply not able to cover everything.

What we have however is an-eadya proto-type of an instrument to ‘watch’ the way governments,

international health agencies, macro-economic rules and structures, multinationals and banks act and
perform in either improving or worsening health and health inequities at the global level. This report

edition does not provide a complete ‘report’ on all the key events and institutions affecting health
globally, but it provides a foundation for subsequent altemative-world-health reportsWatches-that-we

Watch in 2007. What will be said about the key role and performance of the World Health Organisation

in two years time? Will they-it_be able to improve theiF-its_performance whilst defending itself against
the attacks being made upon multi-lateral institutions by the United States? Will the tobacco
companies have found waySr-po&sibly-with-the-Govert-strppGrVof-SGme-govemment-ageRGiesT-Jo

effectively neuter and undermine the effective implementation of the Tobacco Convention? Will the
World Bank still be undermining public sector health care systems whilst trumpeting their commitment
to poverty alleviation? What will have been the real record of the humanitarian and development
efforts in those areas affected by the tsunami after the massive outpouring of civil society generosity?

Politicians, governments, donor and humanitarian agencies, banks, and multi-lateral institutions need
to be held accountable. They need to be praised when they do well; they need to be told when they
fail; and they need to be caughkoutexposed when they consciously contribute to the problem. Perhaps
by 2007, we would also include an independent evaluation of the role of civil society on all these

concerns.

We hope that this report marks the beginning of an on-going process to improve the-ohecks-and

balances-requlred-to-move-us-faster towards-eliminating-preventable-sufferingaccountability in the

international system and contributes to our wider goals of ‘Health for All- as a right1.

Greetings from PHM Secretariat!

I enclose the foreword having seen and endorsed most of Mike’s suggestions and adding a few of my own (in
blue colour), leaving both of you to make the final changes using your discretion - since you know the full
contents better than I do. I only have three differences from Mike’s suggestions : (1) I don’t think we should
drop the reference to Iraq and its health systems (2) even though I agree that we need not be apologetic

about not having a report about Tsunami, I do feel a line at the end is not enough. One way to introduce it is

to have a sort of STOP PRESS which is often added at the end of a publication to indicate an urgent and
relevant addition to the main text. You could do so as the very last page and introduce the post-tsunami

PHM statement that was recently released at the IHF-WSF 2005, which is attached (may need a little editing)
This way we will indicate that we do respond to current realities even though we may not be able to introduce
it into the publication because of the pressure of publishing schedule. This is just a suggestion. You can get

a larger opinion if you wish. There’s been a positive reaction from many and the statement does raise issues

that would have been part of the chapter if we had been able to included. (3) I think in a spirit of accountability
we should not only focus on politicians, governments, donors, humanitarian agencies, banks and multi-lateral
institutions and leave out NGOs and civil society. We, including the PHM (in spite of all the enthusiastic

efforts by many of us) are as responsible for the present situation as all the other groups. This additional line
as a question would only add to our credibility and transparency and reduce inadvertently any ‘moral high

ground’
I am really very pleased that you guys have managed to keep the apparent deadlines and the publication is

reaching the final stages. My main concern has been the case studies since there have been so few that

were circulated. Did we manage enough finally? Or are we left with heavy textual chapters? I know its
rather late in the process to be asking this - but sometimes better late than never.

All the best for the next few weeks of hard work. I was even considering writing to the three of you to consider
taking over the PHM website as part of the post GHW 1 process - because in many ways the PHM web site
could be the continuation of the Watch process at all levels, balancing perspectives and praxis. Just leaving

you with this thought - we could discuss it at PHA 2.

The thinking is that the Foreword would act as a 'welcome' to the first alternative world health report, and that ft
may be written by a well known figure head. One suggestion that has been out forward is Arundhati Roy - she is a
■.veH-known name who is associated with the global social movement articulate etc. Please can we have some
reaction to this suggestion, and some
alternative proposals. You may also want to suggest that we discard the idea of having a foreword written by a
well-known figure.
The note from the three sponsoring organisations would make the following points:
- briefly describe the genesis of the report
- the process for its production
- to indicate the limitations of the report,
- we should also indicate that we hope that the first edition, represents a building block for subsequent reports that
wiil represent a more representative view on health, and which will form a mechanism for holding the key
institutions related to health to account.

Best wishes for the New Year from the GHWsecretariat and our apologies for not being in contact for a while.
A quick update on our progress:
- the Watch chapters are currently being edited and with fingers crossed they will be at the publishers in March
- a Latin American Health Watch is being put together by our friends in Ecuador, apparently this report is well on
the way
- a UK Watch is being put together by our friends from the Politics of Health Group here in the UK
the GHW is being featured at the Porto Alegre World Health Forum and at the World Social Forum at a Health
and Human Rights workshop (also being attended by Jeff Sachs and Pau! Hunt)
- we are already thinking about the GHW2 and what sorts of things can be done which we have not been able to
do this t/r.e arouno.

We are gearing up for a big year- we will need to get our voice heard amongst the other events this year (G8.
Make Poverty History Campaign, Climate Movement Campaign, Trade Justice Campaign, MDG Review, SDOH
Commission. .. )
Here are some tnings we would like from you io help us do this:

1, It would be most appreciated if you couid review this list of journals (attached)- this is for journals who could
feature a review of the report' and assist in its dissemination. Please let me know if there are any crucial ones
wiss;ng from rhe list (possibly relevant to your geographical are?.). I will need this back asap (ie. by the end of
the week v.ouid be good).
2 Please think about the GHW launch and the possibility of having one in your locality. The launch is scheduled
for July 2Cm in Cuenca, London, Geneva, South Africa, (somewhere) We would also like to have launches in
other places eg. Berne, New York or Washington. Los Angeles or San Francisco. Delhi, Brussels. The Hague,
Berlin, Mexico City, Sydney/Melbourne, Jakarta, Ottawa.. Cairo, Nairobi or Dar es-Saalam and wherever else is
possible L A summary of the report will be produced in Spanish and English but no other languages at this stage. course we (or you) can source funds for translation. Please let me know if you can help with a
launch in your city it does not have to be on the 20th out can be sometime after (eg. a significant public health
conference or event).

3. Dlease also think about a Health Watch for your country. This does not necessarily have to be ready for the
time of cur launch in July but you can think of it as an ongoing project. Let us know.

Since GHW is a joint project of PHM, Medact and GEGA, I think all three should definitely be listed as
authors - not just GHW. [ also feel that inside the report, there should be a clear statement ol collectivity
highlighting ail ‘he three lead organizations. Perhaps there could be a creative integration of these options.

Cur editor’s suggestion is that the GHW should be the author (this is so that for future editions we can ensure that
the report will always be listed under the same author - in case the coordinating orgs change). Or should the PHM

Thanks for the crediting info.
I like your idea for the website - we have included website upgrade in our proposals for
funding. Will have to talk to Mike and Dave about this after the MS is submitted.
Andy has also offered to assist us with the advocacy phase for the GHW. We plan to have a
meeting about this soon. The WHA events will be part of our strategy.

I would be happy to be part of the WHA planning team. No worries about the Bangalore
meeting (although i was looking forward to visiting you all there).
And thanks very much to you and to other PHM people for your support (lots of PHM people
have sent supportive words) - we really appreciate them.
