After 11 September 2001
In December 2000, more than 1400 people from 93 countries met in Bangladesh

at the world's first People's Health Assembly (PHA). They developed a
People's Charter for Health which describes the vision of

Thanks for the PHA Statement I It is really important to have a clear
statement with a good focus on what needs to be done at this time. There
are a lot of shallow articles going around the internet on the issue and
the PHA Statement is indeed very timely and properly oriented.

Medact's response to the bombing of Afghanistan

As an organisation committed to non-violent resolution of conflict, Medact
regrets the use of military force in response to the attacks on New York and
Washington of 11 September.
These were terrible crimes against humanity, and the instigators must be duly tried before an appropriate court of
law. However, we fear that the bombing of Afghanistan will delay rather than promote bringing those responsible to
justice. The attacks will claim more lives, and whether these are Taliban figliters or civilians, they will be regarded
as martyrs and their deaths will result in more terrorism. In the words of Richard Horton, Editor of the Lancet (6
October) Principles of harm reduction are more realistic than false notions of a war on terrorism.'

Congratulations for completing all the background tasks and ensuring
curtain raising of the new website on the PHM anniversary.

This note should have come earlier but got misplaced in the pile. In the
reaional news / initiatives section the classification of countries in
-he Asian region needs some further modification. Nepal, Pakistan,
Bangladesh, Sri Lanka have inadvertently come into India section. Also
Philippines in one case.
Perhaps we should inform our Webmaster of which countries go into South
Asia without India; and South East Asia without China regions.

Otherwise it's an excellent job. Congratulation to both of you. I am

Greetings from Peoples Health Movement Secretariat at CHC, Bangalore!
We have all been deeply disturbed about the matter brought up by Nance
but we have .known about this for a few months and have discussed it at
the steering committee. I would therefore suggest that a largo notice in
bold capitals is put up in the exchange that the new website of the
Peoples Health Movement is www.phmovement.org and no longer pha2000.org
which has been taken over by undesirable elements or something to that
effect - so that we direct all those who are looking for the phmovement
website to the new one and away from the porn site. Perhaps a large
^pnouncement on rhe home page of the website may be necessary as
^Enforcement. You may also have to request all PHM members to change
this website address in all the publications, charters, handouts that
they may have with the past website address. Andrew and Nand will have
to go through each of the items on the website where the old website
address is given and change it systematically. More than three months
ago one of our team members have already brought it to our notice and
there has been correspondence with Andrew, Qasem and other about this.
We will have to do this with confidence and tact to ensure that we refer
people away from a undesirable development without giving that site
undue attention. Do your best.

maintenance of the site. Some specific issues:

° Agreement that NewsBrief will continue as a regular communication tool, and therefore that it is
promoted on the site, with a subscription request form - that increases interaction with the site
s Note Nand's suggestion that the latest NewsBrief is e-mailed out to all the feedback people
° We need to have some discussion about how to get people to add their own material to the site
(and, as Nand points out, who will moderate, edit, repackage this and link back to submitters)
ideas on this please.
® Ravi, note also Nand's willingness to work with someone in Bangalore to advise on how to
maintain the site.

Also, a little advance notice - we've got the WHO Director General debate almost lined up. It will be on Sunday, 19 January
from 3-5pm Geneva time (2-4pm GMT) with live sites in Geneva, London, Delhi, Tokyo, Addis Ababa, Saudi Arabia,
Johannesburg, Washington, Rio/Sao Paulo and also will be broadcast across the internet. It will aslo be rebroadcast over
the WoridSpace facilities in Africa and Asia later, and over the Canal Saude facilities In Brazil later.As of yesterday evening
we had 6 of the 9 candidates agreeing to take part, with a seventh having agreed in principle. We're working on the other
two. i'll send you further details later today or on Monday/Tuesday of the actual site locations and how you can engage with
the process. By the way, I'm assuming that PHM wants to be recognised as a co-sponsor of this event, right?

really help in people feeling a true sense of belonging to the

at other ideas.

Greetings from the PHM Secretariat(Global) at CHC Bangalore!!!!
I had been busy with setting up things both at the CHC office and the secretariat here. It is more or less
settled now. Later on I became busy with the National working Group meeting of PHM India where there

us and helps in easier sorting out the mails. Also u could remove some of ur burden, by directing the
feedback mails directly to comm.phmsec@touchtelindia.net without u having to take the extra trouble of
forwarding the same once again to us (that is, if u dont hv any other purpose behind doing the same)
In one or two days I hope to collect all the press realeases that we hv released from the PHM, to be put
on the website. I now hope to get into the loop faster and sooner. U can now start corresponding with me

An update....

Dear Ravi.
Trust you are well. First of all. thank you for your hospitality
during my visit to Bangalore.

^ologjes for the delayed response - a lot has happened since we
last met.
After leaving you I went to Goa and then on to Mumbai. Wltile in
Mumbai. I went through a complete set of medical tests and then
onto to Pune.

In Pune, I met some people who are just establishing a network
io get education to the outlying areas within the districts of
Maharashtra. I am going to be partnering with them to get
"Health Education to Villages". Its exciting news .... and more
about that in another email.
.After returning from Pune, I got my medical test results and
went through another set of tests which discovered a growth in
my throat - around my vocal chords. I delayed my departure from
L^lia and had an operation done to have this growth removed.

After that. 1 went to Hong Kong to rest and recover. 1 was
unable to speak for a couple of weeks and have just started to
speak a little. 1 am presently in Hong Kong and will be
departing tomorrow for home in COsta Rica [3 day stopover in Los
Angeles].

Hence the delay in my response to you.
Thank you for the invitations for the International Health Forum
and The World Social Forum io be held in Mumbai early January. 1
do not know how long it will take me to recover from my
operation and get my voice back totally and hence am unable to
confirm my participation. I feel that any strain on the vocal
chords would be inadvisable -1 was told by my doctor to keep
quite silent and speak with a low voice for about 3 months.
After that, to see how I feel.
1 will keep you advised of my progress. Thanks for your patience.

Just a note to let you know that I found your visit to PHM secretariat very thought provoking. I appreciate your
frankness and 'down to earthiness’ and I hope that this helps me to focus both PHM action and expectations.
Prasanna is back from the PHM India - Mumbai meeting and we are meeting today and tomorrow to work on
a plan of action for the next two weeks, so that he can focus on PHM website fully for a period to really get to
flbw all the nitty gritty. He will keep in touch with you as this process evolves and I am glad that you have
Wered to continue to support / advise him in the matter.

Hope this is not too much intrusion into your holidays, but do keep in mind the suggestion to consider joining
us between 12th - 15th January 2004 at Mumbai (most probably International YMCA near Bombay Central)
for the International Health Forum on 14th/ 15th and the discussions on communication strategy / website etc
on 12th / 13th. The World Social Forum will be from 16th - 21st January 2004.
The only strategic opportunity for you, apart from the inevitable support to the PHM Secretariat is to meet a
large number of local /national / international resource persons - all at one venue, which may be of some
help to you in your future work in the country. If you feel comfortable with the idea, we could introduce a
communication strategy workshop using ORT I Diarrhea as a case study that you and Andrew could facilitate
at the forum.
These are just some ideas to keep in mind, when your holiday gets over.

HIV/ AIDS is also about politics, trade and profits” says the Asian People’s Charter on HIV/ AIDS,
an initiative of the People’s Health Movement (PHM).

?HM is an international coalition of grass roots movements and health professionals that has a
presence in nearly '.00 countries.
“Hit the disease and the social and political context that amplifies the epidemic fast, hard and
everywhere” said Eva Ombaka, from Tanzania and a member of PHM. She is attending the 57th

World Health Assembly being held at Geneva, from 17th to 22nd May 20C4. HIV/ AIDS is a key issue

that the World health Assembly is addressing this year.
PHM is using the WHA to present and discuss the draft version of the Asian People’s Charter on
HIV/ AIDS. The charter is being considered as a ‘much needed tooHA1 to do a ‘reality cGtiedk> on the
HIV/ AIDS epidemic that is threatening global stability.
“The charter is an amplifier of people’s voices” said Zafrullah Choudhury from Bangladesh and a
member of PHM. “We are calling for urgent action by the UN systems, policy makers and politicians
to address the actual realities of the HIV/ AIDS crisis” he said.

,ook at the realities:
1. In Africa, a whole generation is disappearing in front of our eyes. Poverty and profits over
people’s health have turned HIV/ AIDS into a killing machine.
2. The epidemic is spreading faster in pockets of Centra! Asia because of the ongoing armed
conflicts and military expenditure, drug running and human trafficking.

Children orphaned because of"HIV/ AIDS is taking its toll on the next generation. Time is
running out and we need to act now.
4. Politicians and policy makers in Latin America and Asia have yet to learn lessons from the
tragedy that has unfolded in Africa.

“We know the reasons for these deaths. We know their names and addresses. The international
community will never win the battle against HIV/ AIDS unless they address its political, cultural,
economic and social dimensions” said Mwajuma S. Masaiganah from Tanzania. She was referring to
the growing clout of Multi National Drug companies who have become the major stumbling block in
ensuring life for the millions of people infected and affected with HIV / AIDS.
In 2004, there are over 40 million people worldwide infected with HIV. 75 % of them are from
Africa. Life expectancy in Sub Saharan Africa dropped by 15 years due to HIV/ AIDS. In 16 worst
affected countries in Africa, life expectancy dropped by 50 %.
It is in this complex political and social reality, PHM is using the Charter as a rallying point to call
the global attention and give a people’s perspective to HIV/ AIDS crises.

The draft charter is being discussed and debated at various levels- local village meetings to
international forums and the World Health Assembly. Various constituencies are extending their
solidarity- from positive people’s network, village health workers to medical professionals and
campaigners working on access to health care and medicines and women’s rights.
The charter is also supported by various international agencies like ActionAid International, Churches
Action for Health, Italian Raoul Follereau Association (AIFO), Italy, WEMOS, MEDACT, Equinet
(South Africa), Global Equity Gauge Alliance and Health Action International. National Front in
Defense of People’s Health, Ecuador is one of the recent bodies that extended their solidarity for this
charter.
;e final version of the charter will be released at the International AIDS conference scheduled to be
held in Bangkok in July 2004.
The Charter, while demanding to place HIV/ AIDS in the context of primary health care, calls for
radical reforms within the approach of governments, UN institutions like WHO and UNAIDS. “The
acid test of WHO’s 3X5 initiative is in its ability to strengthen primary health care systems and
addressing gender issues including violence against women” said Thelma Narayan from India.

The charter also calls for people-based actions like legal action to challenge the exorbitant drug prices
that are a direct violation of right to life and.also to challenge the proposed changes in patent laws
that will make drug prices further unaffordable.
“The lives that have been lost in Africa should not go as waste. Asia, Latin America and rest of the
world should learn from these costly lessons” said Patricia Nickson from Democratic Republic of
Congo and a public health specialist associated with PHM.

Alma Ata declaration. However, the UN body abandoned the primary health care agenda in the later
years. Health systems, including primary health care’, a new WHO document endorses the primary
health care agenda. It is a welcome return to the basics.
Trassroots movements like the People’s Health Movement (PHM) offer a cautious welcome, but say
this is not enough.

The apex UN health body’s new ‘road map’ that is being presented during the ongoing World Health
Assembly endorses the importance of primary health care- something that grass roots movements like
the PHM has been demanding ail these years.

Consider the positive elements in the WHO document:

It endorses primary health care and calls for a wholistic approach.

It calls for participation and inter sectoral coordination.

It reiterates the responsibility of governments and suggests decentralized action.

4. It suggests the importance of public health orientation to health systems development.
“This is a progressive and realistic move” said Maria Hamlin Zuniga of PHM. “It is good at its
vision but weak in its strategies” she said. However, “The new ‘road map’ for primary health cane
misses the current political and economic realities” she added.

The WHO is completely silent on the disastrous effects of the policies dictated by the World

The document is completely silent on the negative role of pharmaceutical, tobacco, alcohol,
military, baby food and other industries that negatively influence the health of the people.

WHO misses the point when it comes to the impacts of wars, conflict, militarization and
occupation on health and health systems.
“Three key developments that we observe in this document are the emphasis it gives on the
government’s responsibility, reference to health system determinants and use of the ‘primary health
care lens’ to track health strategies”, said Ravi Narayan of PHM. “However, it is absolutely necessary
that WHO strengthens its research capabilities on the social determinants of health so that it can feed
appropriate information to member nations who thus can make informed decisions” he added.
“The resolution mentions the need to integrate disease-control programmes such as the new global
funds with health care systems, but offers no ideas about mechanisms to do this. We need a code of
practice, monitored by WHO, to ensure that disease control programmes do no harm to health
sendees in develooing countries” said Mike Rowson of MEDACT, UK and a constituent of PHM.

However, PHM expressed serious concerns over the fact that the relevance of Primary Health Care
was not reflected in the speech delivered by the Director General of WHO 18tn May.
The People’s Charter for Health, the guiding spirit of the People’s Health Movement and the largest
consensus document on health, addresses some of these glaring omissions from a people’s

People’s Health Movement will keep up the pressure on WHO to ensure that the UN body is not just
a pawn of the industry. PHM calls for creating space within WHO for strong representation of grass
roots movements and ordinary people in policy and project formulation.

The Global Health Watch - an alternative World Health Report - is a new initiative by PHM. This
alternate report is expected to amplify the voices of the grass roots and come up with fresh ideas
about global health policy in the wake of the failures of the last 25 years. The report will highlight the
political, social and economic determinants of health and suggest alternatives to the market-driven
approaches to health and health care.
?EM extends its solidarity for the proposed commission on social and environmental causes of health
inequities and how to overcome them.

Global Polio Eradication Initiative in India

[Abstract: The paper suggests that the inability of the pulse-polio program to achieve

polio eradication in India should surprise no one. This should not be misconstrued as a
failure of a gigantic effort by lakhs of health workers and million of volunteers and

participants in the program in the last eight years. The limitations of the efficacy of the

oral polio vaccine and low routine immunization coverage in several parts of the state
were well known to the WHO and all the other important decision-makers in the country.

Despite this the case for early eradication of polio was aggressively promoted and the

real possibility of failure was underplayed. The expensive gamble has however failed

just as it happened in the past with other eradication programmes like malaria. Neither

the global leaders of this initiative nor the Indian government are taking responsibility for

this failure. The complete lack of transparency and accountability has meant that
enormous amount of public resources, both in terms of money and manpower, has been
wasted in this misadventure. The Global Polio Eradication Initiative illustrates the

process of health policy making in the country and the role of international cooperation in
health in the time of Globalization and Liberalization.)

Executive Summary

The paper is an initial overview of the Global Polio Eradication Initiative (GPEI)

in India. It attempts to address three key issues arising from failure to meet the target of

polio eradication in the country:


How fair was it to impose the ambitious target of polio eradication on India, and

other developing countries; a target which implies that a single case of polio
anywhere would be tantamount to the failure of the entire global initiative? Could

we have not managed better with less ambitious and more feasible target of polio
reduction and control?

If the strategy of polio eradication initiative was faulty to begin with, what were

the interests and influences that have played key roles in aggressively promoting
this strategy in India and globally?

Failure to achieve polio eradication in India despite eight years of intensive

effort. It is argued here that while the magic bullet of pulse polio and Intensive Pulse
Polio Immunization (IPPI) might have contributed to the reduction of the transmission of

poliovirus in India and other developing countries, it has singularly failed to achieve the
eradication of polio. The first deadline of the year 2000 for achieving polio eradication
has already passed. The new deadline of the year 2004 too is very close. In the high

echelon of decision making, it is now agreed that this goal is not going to be achieved in

the near future. Therefore the IPPI strategy has lost its meaning altogether and should
be abandoned immediately.

The paper makes a plea for an independent review of the strategy from 2004 onwards

to decide the future of the IPPI and other SIAs (Supplementary Immunization Activities)

in India. There is an urgent need to learn lessons from the failure of the programme if we
have to avoid repeating the same mistakes. This would also require a review of the lead

role played by the international agencies like the WHO, UNICEF, Rotary International

and Centre of Disease Control (CDC), Atlanta, in order to avoid being misled by them in
future.

Real Magnitude of the Problem: Is polio eradication an overarching priority? It is

also argued that when the WHO launched the global polio eradication program in 1988,

the estimates of polio cases in India (and globally) were perhaps gross overestimates.
Considering that, currently, in India the confirmed polio cases amongst the reported
(Acute Flaccid Paralysis) AFP cases are just 10-20 % of the total, the previous

estimation of the paralyses attributed to polio virus requires a review. This has to be

seen in the context of, how the proponents of the GPEI have admitted that polio
eradication is not a priority for the developing countries. Yet the program has been
justified on the grounds that it may result in small savings to the western countries which

would no longer need to spend on vaccination once polio eradication is achieved
globally. This is an unacceptable reason for over loading the already weak health

systems of the developing countries to this exhausting marathon run. The billions of

dollars spent on this program could have been more efficiently utilized to solve more
pressing problems and in the rehabilitation of the decaying health systems of the
developing countries. It calls for making a realistic exercise of estimation of the

opportunity cost of implementing this program and opportunities lost in not implementing

the more appropriate programs.

Is it possible to repeat the great marvel of eradication of small pox? The high

optimism of repeating the success of small pox eradication with poliovirus too has been

belied. There are several differences in the two situations. First, surveillance to

determine whether and where smallpox virus was present was comparatively simple
compared to polio surveillance and could be rapidly accomplished. The vast majority of
patients with smallpox had a distinctive rash. There were no asymptomatic patients and

no chronic carriers. Thus, it was possible to do this without recourse to a laboratory.

With Polio, there are 200 or more asymptomatic infections for every paralyzed patient.
The only way one can ascertain whether the virus continues to circulate in an area is by
an extended period of surveillance during which a great number of stool specimens are

examined. The second difference is with respect to the efficacy of vaccination.

One

inoculation of smallpox vaccine protected nearly 100% of those vaccinated. In contrast,
OPV in now endemic areas requires at least 3 doses and often 5 or 6 doses to reach

protective levels of 90% for types I and III poliovirus, the predominant paralytic strains.
Thus, in areas, where access and reach of vaccination programs was limited,
vaccination immunity against smallpox could be rapidly increased because of the single

dose inoculation but this is not possible with polio.

Why did the WHO aggressively promote the case for eradication? It is further

argued that WHO over-pushed the case for eradication of poliovirus in India by using the

example of Latin American countries that was not really applicable in the Indian
situation. It also underplayed the real possibility of failures to achieve the goal due to

well known limitations in India and other developing countries. These limitations
pertained to the (i) very low routine immunization coverage in certain states in India and

(ii) the low efficacy of the OPV in providing sufficient immunity in India and other
developing countries. The WHO also overemphasized the possible benefits from the
global eradication of poliovirus and underplayed the negative impacts on the general

health services as well as the risks and costs of the failure of the program. It did all this
against its own wisdom. It narrates how, after the failure of Malaria Eradication Programs

globally, eradication became a 'bad word’ in the WHO, notwithstanding the success

achieved in small pox eradication earlier. It asserts, therefore that the WHO, in its
organizational wisdom could not have, on its own, embarked on a global eradication

program for polio. Based on the information available, it is difficult to figure out what
influences have played a role in WHO for pushing polio eradication as an utmost urgent

priority for the developing countries. We admit our failure in solving this riddle and invite
others to help us in this endeavor.

The paper suggests that GPEI is yet another negative exercise in mismanaging the

health priorities and programmes in developing countries. The UN institutions, their
corporate philanthropic partners and the gullible health bureaucracies, technocracies
and political leaderships of the developing countries, all are equal partners in this futile

and absurd exercise. We also question the right of the G-8 nations and international
philanthropies like Gates Foundation to provide earmarked resources for particular

disease control programs and thereby distorting the health policies, priorities and

programs of the developing countries. This appears to us as the biggest blemish on the
international cooperation in health. Considering that global commitments are involved, it
is necessary for India to play a more proactive role with the governments of the other

SAARC countries and the African countries where the WHO has thrust the program for

the last several years. The paper also calls for learning lessons from the entire GPEI for
the functioning of WHO and the GPPP within the WHO.

The way forward for India. There is a great urgency to make critical decisions for

strengthening the health systems in India. Only this can enable us to strengthen our

routine immunization programme and achieve control of polio in due course. There is no
shortcut to this process. The positive lessons should be drawn from the gigantic efforts

made by lakhs of health workers and millions of volunteers and participants in the
programme throughout the country. The programme should be publicized as a success
in reducing the transmission of poliovirus in the country and not as a failure, just

because some cases are still occurring. The live contact made with millions of poor and
disadvantaged people for the first time by the country’s health systems should be
strengthened for health improvements in other areas and with involvement of the newly
activated civil society.

9.

Right of compensation for those who have suffered from Vaccine Induced

Paralytic Poliomyelitis (VIPP). A considerable proportion of children who have
developed AFP due to polio virus are those who have received 3 or more doses of OPV.
There is need for comprehensive rehabilitation of those suffering from residual paralyses
(due to poliovirus and other causes). In the current strategy these aspects find no place.

The inferences made in this paper are based on available information on the issue. It is
perhaps not possible to avoid hasty conclusions and ideological biases while stimulating
discussion and debate, especially while all the data and information has not been placed
on the table. We invite the readers and the experts in the field to place other facts

and information on the table, challenge the inferences made in this paper and
provide alternative interpretations and explanations.

draft of genuine human stories in midst of all the technical details. I was moved by what was
reflected from their eyes. The entire turn of events was so distrubing that I literally had to take myself

The name Bhopal we almost reflexly related it to 1984-The Bhopal Gas
Disaster of Union Carbide Industry that produced pesticide Sevin. It
also produced.. Die worst ever Industrial disaster in the history of the
world till date....

In my ven’ younger days, I vaguely remember people gluing to the then
omnipresent Black and White TV sets watching grim pictures, discussing
about some catastrophy somewhere in India, way back in 1984. After
almost twenty years, my battle with pesticide, now, draws me to this
magnificent city of Nawabs.
..Today If you happen to pass by the derelict Union Carbide pesticide
factory7 in Bhopal nothing would strike you as out of ordinary. It
looks like and other abandoned factory. Rust has set in and creepers are
growing over machinery. The only sign of life is a bored watchman who
isn’t quiet sure what exactly he is supposed to watch...

>The sign board on the Entrance gate reads "Prohibited area" guarded by
homeguards. It is mandatory' to obtain official written permission from
the District Collector for entry. .After being convinced of my sustained
interest in Environemntal/Pesticide/occupational health and
verification of my identity from my friends in Sarnbavana Trust (Very well known
Bhopal gas disaster campaigners in India, based in Bhopal), the Deputy
Collector signed the written permission orders for me to visit the ill-fated industrial site with copies
marked to at least four departments.

> Armed with the permission order’s from the DC, I visited rhe Union
Carbide Industrial site. .After passing on copy of permission marked to the
Home guards on duty on the site, the gates were thrown open for an
experience, which was to me, like undersea-diving to watch and feel the
ruins of TITANIC.

CHILLING ACCOUNT OF THE BHOPAL GAS DISASTER AT UNION CARBIDE
INDUSTRY AT BHOPAL ON 3 DEC 1984.

ONE SUNDAY NIGHT....

Late on Sunday evening, December 2, 1984 during routine maintenance
operations in the Methyl Iso Cyanate (MIC) plant a large quantity of
water- entered on to the storage tank containing 60 tones of MIC. Tltis
triggered off a runaway reaction resulting in tremendous increase of

temperature and pressure. Little before midnight, a deadly cocktail of MIC,
Hydrogen Cyanide, Mono methyl amine and other chemicals was earned by
northerly wind io ihe neighboring communities. Over the next couple of
hours close to 40 tones of chemicals spread over the city of the about
one million peoples covering an area of 40 sq km.

People woke up surround by poison cloud so dense and searing that they
could hardly see. With their eyes stinging and their throat burning,
hundreds of thousands of people ran screaming for their lives. As they
gasped for breath, the effects of gas grew even more suffocating. The
gases burned the tissues of their eyes lungs and attacked threir nervous
system. People lost control of their bodies. Urine and faeces ran down
their legs. Some began vomiting uncontrollably. Others were wracked with
seizures and fell dead. The gases irritated peoples' lungs into
producing so much fluid that their lungs were filled with it "drowning" them
in their own body fluids.

People had no way to knowing it but they could have been safer tunning
against the wind towards factory or by simply covering their faces with
wet cloth. Nobody told them. The factory’s siren had been switched off.
Not until the gas was upon them, filling their mouths and lungs, did
people know of their danger. Union Carbide’s plant manager was woken at
1:30 am as the gas was still pouring over the stricken neighborhoods.
The first official notification by Union Carbide management was done at 3
am when they informed police officials that the leak had been plugged.
In fact it had not been, there just wasn’t any gas left in the tank. By
4:30 am the manager was in this office telling reporters that his
safety measures were the best in the country. Another Carbide official was
telling newsman "Nothing has happened. Can’t you see us Alive? Barely
100 yards away, dead bodies lay on the ground outside the factory gates.
A CITY OF DEATH....
It was a massacre. Dawn broke over streets littered with corpses.
Though official figure was 1,600, the exact death toll in the immediate
aftermath is not known and possibly will never be. Many families living on
the wayside without any address were wiped out. While it was still
dark, hundreds of dead bodies were carried away in the government vehicles
and dumped in nearby forest and a river. In the days after the leak
6000 shrouds were distributed by religious organization and many were
buried or cremated in the condition they were found Union Carbide said that
reports of its victims deaths were greatly exaggerated . It said the
leak had killed only 1408 people. The company complained that if only
people had kept calm and not run away, they would probably have survived.

Doctors on the duty in the emergency ward at Hamidia hospital were
besieged by the thousands who came choking and gasping for breath, had no
knowledge of how to treat them. They called Union Carbide’s medical

officer who said that that MIC was like tear gas, an irritant but non poisonous and the treatment was to wash with water. Not before long it
was obvious that the medical officer had been lying. The hospital was
filled with the dead and dying. By mid morning the hospital mortuary was
overflowing and soon entire hospital compound was covered with dead
bodies. Froth dry in their mouths and the horror of poison cloud in their
still eyes. By noon cremation grounds had rows of smoldering pyres and
the burial grounds old graves had to be dug to accommodate fresh
corpses.

Many of the people who survived the terrible night died slow and
painful death in the years following the disaster. In fact official figures
so that more people have died in the subsequent years the and the
immediate aftermath. To this day 10-15 people are dying every month from
exposure related complication. The current death toll is well over 16,000
and counting.

THE UNENDING AFTERMATH...
Medical research earned out by official agencies have established
that the half a million people who were exposed to toxic gases had poison
circulating in their blood str eam. The toxins caused damage to their
eyes, lungs, kidney, liver intestines, muscles brain and reproductive
immune systems, 40% of the women from the severely affected communities
who were pregnant at the time of disaster, aborted. Anxiety depression,
insomnia irritability are common among the affected people. Chromosomal
aberration were detected in an unusually large number of survivors
indicating the likelihood of congenital malformation in future generation
of survivors. Union Carbide continues to treat medical information on
the leaked chemicals as a trade secret. Till today vety little, is known
about how to treat chronic exposure induced diseases.

Like Hibakushas of Hiroshima, people marked by Carbide, poisons have
to suffer social ostracism Young Bhopali women find it hard to get
married, and many women have been deserted by their husbands because of
their inability conceive. Impotence among men is a pain secretly canied
and drowned in alcohol. Maleness is also seen to be lost, men who can no
longer sustain their family. The company succeeded in winning a
settlement in 1989 with the Indian government that passed a law to become the
sole representative of the victims. Over 95?4> of the people most of whom
are likely to be sick all their lives have received Rsl 5,000 (
500USS)wltich is not even sufficient to pay for their medical bills for five
years

A LEGACY OF INDIFFERENCE..
Warren Anderson, the Chairman of the Corporation charged with homicide
jumped bail and is yet to face the ongoing court proceedings. Its mort
than six year's since the Bhopal District courts issued an arrest
warrant against him but the Indian government is yet to move on iris
extradition from US. Charges against the rest of the officials have been diluted

so that the disaster, legally speaking has been made to look like an
ordinary car accident.

The toxic legacy of Union Carbide continues to haunt the communities
in the neighborhood of the factory. Huge quantities of chemicals
wantonly dumped in and around the factory' have found their way into ground
water. Hie community hand pump bring forth water laced with poisons
damaging to the liver, kidney and lungs. At least two of these chemicals are
known carcinogen

THE STRUGGLE TO SURVIVE...
Faced with corporate machination of liability evasion, a government
that considers their lives expendable and legal system that persistently
denied them justice, the survivors of Bhopal continue to cany on with
the longest struggle ever. Most of these valiant ■victims are women.
Many have cast off their Veils, discarding the roles imposed by tradition.
Hundreds gather every' week in public park, thousands rally on the
streets an often take part in the protest t marches to New Delhi, the
national capital 750 kms away. On several occasions their demostrations have
been attacked by policemen leaving bleeding heads and fractured limbs
behind. In defiance they chant, " We are the women of Bhopal, we are
not flowers, we are flames"

THE CAUSES OF DISASTER IN RETROSPECT...

The immediate cause of the disaster are related to the cost cutting
drive initiatives by Union Carbide Corporation from its headquarters in
Danbury, Connecticut in 1980. The moves directed at enhancing profits
included: reducing the number of personnel, use of low quality
construction material, cutting down on vital safety measure and adoption of
hazardous operating procedures.
Workers had to pay the cost of these economy measure with their life,
health, jobs. Between 1980-1984 the work crew of MIC unit was halved
from 12 to 6 workers and maintenance crew' from six to two workers. On
December 26, 1981 a plan operator was killed by phosgene gas leak. Another
Phosgene leak in January 1982 severely injured 28 workers and in
October of the same year MIC escaped from a broken valve and four workers
were exposed to the chemical.
On the night of the disaster water being used for washing the lines
entered the tank containing in MIC through leaking valves. The
refrigeration unit which should have been kept the MIC close to zero degrees
centigrade had been shut off by the company officials to save on
electricity bills [ confessed by Kamal pareek, Safety officer, Union Carbide
India Ltd.]
The entrance of water in the tank, full of MIC at ambient temperature,
triggered off an exothermic run away reaction and consequently release

of the lethal gas mixture. The safety system, which in any case are not
designed for such a run-away situation, were non-functioning. The
scrubber designed to neutralize any escaping gas by spraying caustic soda
was empty-. .And the flare tower meant to bum off any gases from scrubber
was under repair.

By all accounts, the December 1984 disaster has been followed by
medical disaster of unmatched proportions. Many stories pieced together
mainly from newspaper reports underline the moral, ethical and quite
possibly legal transgression of principal author of this continuing medical
disaster...

end to worries

dear RN/TN mentors.

Greetings from Raipur.
Finally I have joined sundar. my immediate job here is to put to gehter block level survey they have
done in 15 block of health care servics provision and availability, and second job would be
prescription analysis for essential drug list programme.
Today evening Sundar took me to attend a liigh powered meeting called by Health Secretary and
attended by Director DHS and other directors. I enjoyed it, this sec is Surgeon and has great sense of
humour, kept us laughing all the time.

Raman was telling me that he had good meeting with you and you had some very nice words for me
and that you were concerned about me. I have now put an end to all the concern and worries of my
near and dear- ones.... While I was in Delhi I met Sampath.He has offered the UNDP integrated
diseseas surveillance job. My boss their would be Reuben Samuel. I have accepted it. Reuben has
called me to discuss my roles and responsibility for my job there from April. I am visiting
Bhubneshwar in the second week end on 13 and 14th.
