In this fifteenth edition, the authors look at the environmental effects of continuing
economic growth as the economy outgrows the earth's ecosystem. The book describes the
consequences of the collision between the expanding economy and the earth's natural
limits: shrinking forests, falling water tables, eroding soils, collapsing fisheries, rising
temperatures, and disappearing species.

Long-term implications of low fertility in Kerala, India. Asia-Pacific Population

The State of Kerala is about two decades ahead of India as a whole in achieving the
replacement level of fertility. Currently the total fertility rate is about 1.7 children per
woman compared with 3.0 for India. Because of the remarkable progress made by Kerala
in its demographic transition, its demographic trends in the first half of the twenty-first
century will be dramatically different from those of the second half of the twentieth
century. This article provides a basis for policy makers to use in preparing for those
changes, which include population ageing in the face of inadequate social security
systems, impacts on young people, education and employment, and migration.

Revising the proximate determinants of fertility framework: What have we learned
in the past 20 years?. Studies in Family Planning. 29(3): 255-267, September 1998.
The proximate determinants of fertility framework, developed in its current form by
Bongaarts, has been used extensively by researchers for the past 20 years. Since the initial
framework was developed, a wealth of new survey data on the proximate determinants has
become available. This article reviews the new data and past experiences and suggests
modifications to the framework that would take advantage of this experience. The major
modifications suggested are 1) the use of sexual activity rather than marriage to indicate
exposure to pregnancy; 2) a revision of the sterility index to measure infecundity from all
causes; 3) a revised index of contraception that accounts for the fact that users of
sterilization may become infecund before age 49; and 4) a revised definition and estimate
of total fecundity.

Coining of age: from facts to action for adolescent sexual and reproductive health.
Geneva, WHO, 1997. 179 p. (WHO/FRH/ADH/97.18)
This guide has been developed to enable programme planners and managers to carry out
a rapid situation analysis of adolescent sexual and reproductive health so that programmes
may be designed or improved on the basis of sound, up-to-date information. It provides
guidance on the actions that should be undertaken in three phases over 5-6 month period:
planning the situation analysis, doing it, and using its findings. Various methods and
procedures are proposed which may be freely adapted and flexibly implemented according
to the needs and resources of each setting. The guide focuses on the sexual and
reproductive health of adolescents in recognition of the fact that the initiation of sexual
activity is an almost universal feature of this second decade of life and becoming an adult
sexually is so central to their overall health.

Private health sector growth in Asia: issues and implications. New York, John Wiley
& Sons, 1997. 254 p.
This book is based upon the papers commissioned by the Asian Development Bank for
the Second Regional Conference on Health Sector Reform held in Manila in March 1996.
The book focuses on private sector growth in the health sector and collaboration between
the public and private sectors in achieving national health objectives in Asia. It examines
policy responses to the expanding private sector provision of health services in six
countries: India, Indonesia, Republic of Korea, Philippines, Thailand, and Viet Nam.
These issues are reviewed in four sections dealing with (1) an overview of policy issues in
private sector provision of health care; (2) demand side factors and financing options; (3)
supply side factors and payment mechanisms; and (4) quality of care. The book concludes
with a summary of these issues and discussion of future health sector reform in the region.
The papers in this volume reflect comments gleaned from policy makers, health service
providers and researchers from the twenty countries represented at the conference.

The effect of child mortality on fertility regulation in rural Bangladesh. Studies in
Family Planning. 29(3): 268-281, September 1998.
This study analyzes longitudinal data from Matlab, Bangladesh, to examine the impact of
child mortality on subsequent contraceptive acceptance and continuation. The strong
negative impact is found to attenuate with family size, indicating a "replacement effect."
An "insurance effect" is observed as contraceptive acceptance and continuation were
negatively associated with the number of previous deaths of children. Couples seem to find
contraceptive use acceptable if the child who dies is one of a large family. Potentially,
contraceptive use could be acceptable for spacing after a child in a small family dies.
Family planning programmes can help to reduce fertility and maternal and child health
risks substantially by supplying appropriate methods to those couples who have
experienced a young child’s death; to be most effective, methods should be supplied
immediately after the child's death.

Too young to die: genes or gender?. New York, United Nations, 1998. 260 p.
(ST/ESA/SER.A/155)
This study consists of 10 chapters that explore a broad range of mechanisms, ranging
from the biological to the social and economic, that influence sex differentials in childhood
mortality. The mechanisms considered affect mortality differentials either directly, as in
the case of the nutritional status of boys and girls or the differential use of health care by
sex of child, or indirectly, as in the case of maternal education and decisions regarding the
intra-household allocation of resources. The study also provides a historical account of the
evolution of the sex differentials in childhood mortality in today's developed countries
during the nineteenth and early twentieth centuries. Another major contribution is its
measurement and analysis of the levels and trends of infant, child and under-five mortality
for boys and girls during the 1970s and 1980s. Such analysis allows the identification of
the countries and regions in which girls have a survival disadvantage compared to boys,
and permits an objective assessment of the magnitude and extent of excess female
mortality in childhood.

An integrated multimedia campaign featuring family planning messages saturated the
900,000 person city of Bamako, Mali, for three months during the spring of 1993. With
traditional theater and music, family planning messages were repeatedly broadcast on
radio and television that conveyed information about modem contraceptive methods, the
need for male sexual responsibility, the health and economic advantages of family
planning, the need for communication between spouses, and that Islam, the predominant
faith of Mali, does not oppose family planning. A separate sample pretest-post-test quasiexperimental research design was used to evaluate the effects of the campaign and
exposure to specific messages on changes in contraceptive knowledge, attitudes, and
practice.

Saad, Paulo Murad.
Recent migration in the State of Parana. Curitiba, Brazil, Paranacidade, 1998. 29 p.
(Paranacidade technical series; no. 4)
After a period of intense population growth, the State of Parana experienced huge
population losses during the 1970s. Recent studies refer to a timid reduction in the losses
during the 1980s, but little was known until now about the migration movements in the
State during the 1990s. The present study substantiates the declining trend of population
losses found in previous studies, by showing an incontestable reduction of the State's
migration balance deficit during the first half of the 1990s. Moreover, the analysis
indicates the attenuation of previously important migration flows, and suggests the
emergence of a new migration profile in the State in which the eminently young migrant
with no professional qualification, typically from the rural zone, is gradually replaced by a
more mature and qualified urban migrant.

Linkages between population and education: a technical support services report.
New York, Population Division, Department of Economic and Social Affairs, United
Nations, 1997. 42 p. (POP-TSS-97-6)
This paper clarifies the impact of education on population variables. The review gives
particular attention to the linkages between education and fertility, although its
relationships with mortality and migration are also mentioned. Among the socio-economic
factors that affect the three demographic processes, education is a key determinant,
although its impact is enhanced or reduced according to the social and cultural conditions
of every countries. Educated individuals are better equipped to safeguard the survival of
mothers and their children; they are also in a better position to take appropriate decisions
concerning their migratory options. The links fertility are analyzed through the impact on
fertility preferences, as well as on nuptiality, breastfeeding and use of contraception. The
review of the literature indicates that education is the most solid and consistent variable to
increase the demand for family planning services. The review is accompanied by an
appendix that contains relevant provisions of the ICPD Programme of Action.

This book looks at the history and evolution of policy commitment underpinning four
governments' population efforts over the past half-century. What are the essential
elements of political commitment that will sustain a successful population programme?
Can the core features be teased out in a cross-cultural comparison? How should these
features be assessed? Have foreign influences - primarily from the United States - really
been motivating and sustaining factors? Does the gap between elite and public perceptions
of population issues affect policy? These are some of the questions that this book explores.

Cause of death and active life expectancy in older population of the United States, by
Mark D. Hayward, Eileen M. Crimmins and Yasuhiko Saito. Tokyo, 1998. 192-213 p.
(NUPRI reprint series; no. 70)
Reprinted from "Journal of Aging and Health," Vol. 10, No. 2, May 1998, pp. 192-213.
This study investigates how the major causes of death end active and inactive life among
older Americans. Based on data from the Longitudinal Study of Aging, a multistate life
table model of individuals' age-graded mortality and disability experiences reveals that
many, and sometimes most, of the deaths occurring among the elderly population happen
when people are active relatively close to death. This pattern is especially evident for men.
Despite differences between active and inactive elders of a given age in the length of
inactive life, few differences occur in the causes of death. Simulations show that many of
the gains in life expectancy via the reduction in major diseases are felt in terms of
increased active life, especially among the young-old and men. These results point to
possible mechanisms in which improvements in mortality extend active life.

The long-range population projections presented in this report cover the period from 1950
to 2150. A total of seven projections for each of the eight major areas of the world are
considered. The variants are distinguished by their assumptions regarding future scenarios
in total fertility rates. The range of potential demographic outcomes underscores the
difficulty of focusing on any particular scenario and also highlights the critical importance
of current policies and actions for the long-range of the world population.

series K, no. 14)
The present handbook provides a framework for developing and disseminating, in the
form of publication, a minimum set of statistics and indicators on women's and men's
position in society. The first chapter describes the institutional arrangements and resource
requirements for the development of a gender statistics publication as a project that could
be either independent or an integral part of a well-established publication programme. The
second and third chapters explain how to prepare and use available data and generate
indicators on the basis of critical gender issues and concerns identified through user
producer consultations. The fourth provides details on tools for presenting statistics and
indicators and on how they are combined to produce a user-oriented publication. The fifth
chapter reviews steps in printing a publication and different ways of disseminating it.
Other types of statistical outputs, such as wall charts and booklets, are also described.
